Return here after reviewing the Program Descriptions to express your interest in your selected program from the Business Leaders Bundle using the following form. Conditions: 1. If government funding assistance is available on ANY of these programs, then the use of these funds will be included in the formal quotation, and the fees including and excluding the funding will be outlined in the quotation. 2. A committed purchase order must be received no later than 5.00pm 15th May 2009. 3. A formal quotation, consisting of the agreed deliverables and outcomes and a fixed price must be provided before any services can commence. 4. Payment terms and conditions and delivery of outcomes will be provided in the formal quotation and must be agreed to prior to the commencement of the delivery of any services. 5. Services must commence prior to 30th June 2009 and must be completed no later than 31st January 2010 unless otherwise agreed to by both parties. 6. Terms and conditions for this offer will be contained in the formal quotation and must be fulfilled PRIOR to the commencement of the FREE mentoring services or initial environmental consultation. 7. FREE mentoring consists of a maximum of 6 hours of services delivered at your premises between 1st August 2009 and 30th April 2010. The FREE mentoring sessions must be delivered in a maximum of 2 on-site sessions. 9.
